RNP Outreach Navigator Job Description 

Salary Range: $3,180.31 – $3,997.84

Hiring Salary: $ 3,180.31 – $3,576.83 per month DOE 

Benefits, which include: Eligibility for health, HRA Veba, Health Saving Account, voluntary dental, and vision coverage, PERS retirement plan options, subject to plan terms. Eligibility for company-paid benefits such as life insurance, short and long-term disability, subject to applicable waiting periods. 37.5 hour work week, 11 County paid holidays. 2 Floating holidays each year, vacation and sick leave accrued from first day of employment, 6% step increase every 2 years. Paid twice per month on the 10th and the 25th of each month.

JOB SUMMARY:

The Outreach Navigator will provide screening, outreach, and engagement to individuals referred to the Recovery Navigator Program. The Outreach Navigator will act as the contact point for all referrals to program case management. This position must ensure effective and efficient communication as well as collaboration between all partners involved in referring and receiving referrals for the program. The Outreach Navigator will conduct community outreach as needed to engage referred individuals who have yet to connect with their assigned Case Manager. The Outreach Navigator must develop and maintain positive, collaborative relationships with all Recovery Navigator Program partners and other service providers to best serve program participants.   This is a non-exempt position, subject to a collective bargaining agreement.

 

SUPERVISORY RESPONSIBILITIES:

Work is performed independently under the direction of the Clinical Supervisor or designee.

ESSENTIAL FUNCTIONS: 

  1. Must work effectively with the client, public, community partners and other employees.
  2. Be point of contact and primary liaison with law enforcement, community, and others making Recovery Navigator Program referrals.
  3. Provide initial screening and engagement with referred individuals.
  4. Manage and maintain information regarding referrals in database.
  5. Provide street outreach to engage referred individuals.
  6. Follow up with individuals who may show signs of disengagement from services.
  7. Assist with care coordination, to include psychoeducation.
  8. Maintain working knowledge of peer specialist and recovery supports.
  9. Articulate their own recovery, resilience, and lived experience in ways that are relevant to the obstacles faced by individuals referred to the program in order to build rapport and engagement.
  10. Assist participants in gaining access to a variety of programs (e.g., Medicaid, SSI, housing, et); assist participants in navigating barriers to access.
  11. Assist with developing a person-centered, individualized service plan based upon individual strengths.
  12. Develop and maintain a working relationship with the emergency response, law enforcement, fire departments, social and health provider network, and other community organizations which may support participants.
  13. Provide advocacy and support for participants within the criminal justice system.
  14. Provide crisis response support, within program standards for Peer Specialist, to include after-hours.
  15. Attend stakeholder work groups and committees to represent the experiences of program participants.
  16. Complete all required trainings within identified time frames.
  17. Complete all required reporting within identified time frames.
  18. Be familiar and follow all state and federal laws relevant to position and scope of responsibilities.
  19. Be familiar and follow applicable departments and all County Personnel Policies and Procedures.
  20. Be familiar with grant and contractual requirement relevant to position.
  21. Perform other duties and assignments as requested by the RNP Supervisor or Clinical Director.

KNOWLEDGE AND ABILITIES: 

  1. Knowledgeable of harm reduction strategies and common mental health and substance use issues.
  2. Able to effectively support culturally diverse, and socioeconomically disadvantaged, individuals with disabilities, and populations disproportionately impacted by the criminal legal system.
  3. Familiar with outreach engagement approaches, service referral and available community resources, recovery supports and organizations for those with substance use and co-occurring disorders; the regional system of care including mental health agencies and substance abuse and recovery services; Community resources, housing providers; federal and state entitled benefits, and other programs individuals may qualify.
  4. Able to exercise tact and discretion in obtaining cooperation of others.
  5. Ability to maintain a professional demeanor and effectively communicate in high stress situations.
  6. Able to exercise discretion and independent judgment in all areas of job performance including adherence to appropriate professional boundaries and strict confidentiality practices; relevant Federal and State laws, rules and regulations, to include HIPAA and 42 CFR part 2.
  7. Able to work both independently and as a member of a team.
  8. Possess effective time management skills in coordinating daily activities, appointments and completing necessary paperwork.
  9. Operate standard office equipment, (e.g., computer, cell phone, agency owned vehicles, et.).

EDUCATION AND EXPERIENCE: 

  1. High school diploma or equivalent.
  2. Must have lived experience, defined as a current or past recipient of behavioral health services and/or with current or past involvement in the criminal legal system. Must be in active recovery, and able to provide appropriate case management and support services to program participants without impairing self ability to remain in active recovery.
  3. Experience working with individuals with complex life issues and/or behavioral health conditions preferred. Experience providing field-based outreach and engagement highly desired.

 

LICENSES AND OTHER REQUIREMENTS:

  1. A Washington state Department of Health Agency Affiliated Counselor certification, preferred.
  2. Possess or actively demonstrate pursuit of Washington State Peer Specialist Certification, within 90 day of hire.
  3. A valid National Provider Identification Number (NPI), preferred.
  4. Valid driver’s license required
  1. Must successfully pass a background, a driving record check and a pre-employment physical.
  2. Is not a State or Federal (OIG) and System for Award Management (SAM) excluded provider, in any capacity for the Medicare, Medicaid, and all Federal health Care programs.
  3. Must be in active recovery, and able to provide appropriate support services to program participants without impaired ability to remain in active recovery.

 

WORKING ENVIRONMENT / PHYSICAL ABILITIES:  Work is performed in office and in the field with travel expected throughout the county. Hand-eye coordination and fine manipulation skills are necessary to operate computers and various office machines, with occasional lifting of up to 50 pounds.  Because of the nature of this position, duties must be performed during regular hours within the department or as designated by the Counseling Services Director, to include flexibility to work weekend and evening hours.  Target working population are individuals with mental health, substance use and related behavioral health needs.
